North City District Nursing Team
The District Nursing Team based at Newnham Walk Surgery serve our practice. They come to the surgery on a regular basis and meet with the whole team. A messaging service is available via reception at Bridge Street Medical Centre.

Midwife

Your Community Midwife is Ruth Todd and she is in the surgery for antenatal clinics on Wednesday afternoons but works in the community doing home visits

Ruth Todd is part of Scarlett Team which is based at the Fawcett Childrens’s Centre. Summary Care Record – Have you made your choice?

Summary Care Record (SCR) is an electronic record of important information about a patient’s health. It will initially have information about current medications, allergies and any bad reactions to medicines. Additional information may be added over time if a patient gives their consent.

We are planning to upload Summary Care Records from 5th September 2011.

If you do not want a SCR created please come into the practice before the 5th September and ask for the ‘opt out’ form. After this date the records will be uploaded onto the system.

For further information, please ask reception. You can also phone the Summary Care Record Information Line on 0300 123 3020 or visit the website at www.nhscarerecords.nhs.uk. Alternatively contact the Patient Advice and Liaison Service (PALS) at the PCT on 0800 279 2535.

 

Data Protection Act

The practice complies with Data Protection and Access to Medical Records legislation. As patients of this medical centre, you are entitled to access your medical record, and certain information about the running of the medical centre. If you wish to access such information, or have questions, please contact the Practice Manager. Please note there may be a fee for this service.

Patient records are currently held on both computer and paper files. All practice staff involved in your care have access to records, although only medical personnel may enter consultations. Confidentiality is a priority, and every precaution is taken to ensure confidentiality is maintained at all times.

 

Zero tolerance

Along with NHS policy, the medical centre is unable to tolerate members of the public being verbally or physically threatening towards practice staff. It may be appropriate, after discussion, to ask a patient to register with an alternative practice.
