If you take systemic Hormone Replacement Therapy (HRT) you will need an annual review before we can re-authorise your prescription.

This online HRT check aims to make it quicker and easier for you to carry out this review. If you are having any issues with or concerns about your medication, then please do book an appointment with your named GP if you would like to discuss anything further.

Unfortunately supply continues to be an issue for many forms of HRT; please ask to speak to our pharmacist if your medication is suiting you but you are having trouble obtaining your prescription due to stock issues.

To fill in this form you will need your current height, weight and blood pressure. If you do not have the equipment to measure these at home you are welcome to drop into the practice to do so (no appointment needed).

The HRT check will be sent to the practice team and your request will usually be processed within two business days. You must complete every part of this form for us to be able to reissue your prescription safely.

About the Medication

If you have a uterus, you will need combined HRT to protect your uterus from unopposed oestrogen. Unopposed oestrogen can cause cancer of the endometrium, the lining of the womb. Your HRT must be composed of BOTH oestrogen and progesterone so the womb is protected.

The progesterone arm of your HRT may take one of the following forms.

Ages 25-49 should have a smear test every 3 years. Ages 50-64 it is every 5 years. 65 and over will only need one if 1 of your last 3 tests was abnormal
